South Australia launches Adolescent Transition Care Framework and Action Plan 2026–2030

SA Health has released the Ado­les­cent Tran­si­tion Frame­work and Action Plan 2026 – 2030 (PDF 2.7MB), which aims to improve how young peo­ple aged 12 to 25 are sup­port­ed when mov­ing from pae­di­atric to adult health services. 

This new frame­work was shaped through exten­sive con­sul­ta­tion with young peo­ple, includ­ing the CEIH Youth Advi­so­ry Group and mul­ti­dis­ci­pli­nary clinicians. 

Young peo­ple and clin­i­cians iden­ti­fied key bar­ri­ers that make tran­si­tion hard­er than it should be. In response, the frame­work pro­vides prac­ti­cal, evi­dence-based strate­gies to improve access, reduce delays and sup­port a more con­sis­tent expe­ri­ence for young peo­ple and their families.

Devel­oped in part­ner­ship with the Ado­les­cent Tran­si­tion Care Statewide Clin­i­cal Net­work, the CEIH, and the Depart­ment for Health and Well­be­ing, the frame­work pro­vides clear guid­ance to sup­port bet­ter tran­si­tion out­comes for young people.
