A more connected response for people experiencing homelessness

A six-month pilot is bring­ing health, hous­ing and home­less­ness ser­vices togeth­er to bet­ter sup­port peo­ple expe­ri­enc­ing home­less­ness with com­plex health needs. For Lau­ra, a shared plan helped ser­vices respond to sup­port her move into accommodation.
