Notifying termination of pregnancy

Information on the Termination of Pregnancy notification form, reporting requirements and submission options.

The Ter­mi­na­tion of Preg­nan­cy Act 2021 com­menced on 7 July 2022 in South Aus­tralia, pro­vid­ing a mod­erni­sa­tion of ter­mi­na­tion of preg­nan­cy law and prac­tice and improv­ing the effi­cien­cy of health ser­vice pro­vi­sion and access, par­tic­u­lar­ly in region­al, rur­al and remote areas.

In line with these reforms and changes, the fol­low­ing meth­ods for report­ing oblig­a­tions for reg­is­tered health prac­ti­tion­ers, hos­pi­tals and pri­vate day pro­ce­dure cen­tres to the Chief Exec­u­tive (as per Ter­mi­na­tion of Preg­nan­cy Reg­u­la­tions 2022) will be required.

The new Ter­mi­na­tion of Preg­nan­cy noti­fi­ca­tion form (PDF 1MB) is required to be com­plet­ed for all ter­mi­na­tions of preg­nan­cy in South Aus­tralia. This form replaces the pre­vi­ous green ter­mi­na­tion of preg­nan­cy form (often known as COR19):

  • the new Ter­mi­na­tion of Preg­nan­cy noti­fi­ca­tion form can be filled in dig­i­tal­ly using a com­pat­i­ble PDF read­er (e.g. Adobe Acro­bat Read­er) which enables the form to be saved and sup­plied to the Preg­nan­cy Out­come Unit via a secure link embed­ded in the form
  • the new form can also be print­ed and sub­mit­ted via Aus­tralia Post (see form for postal details).

Ter­mi­na­tion of preg­nan­cy ser­vice providers locat­ed with­in South Aus­tralia are required to com­ply with infor­ma­tion pro­vi­sion under the Ter­mi­na­tion of Preg­nan­cy Act 2021 regard­less of the usu­al place of res­i­dence of the per­son hav­ing the ter­mi­na­tion of pregnancy.
