Strengthening care through new networks and Priority Focus Areas

The CEIH is imple­ment­ing its revised mod­el for Statewide Clin­i­cal Net­works (SCNs) fol­low­ing a review com­plet­ed in 2025

The team is busy set­ting up new SCNs, tran­si­tion­ing exist­ing Com­mu­ni­ties of Prac­tice into the CEIH, and sup­port­ing the SCNs already in place. 

SCNs bring togeth­er health pro­fes­sion­als, con­sumers and sub­ject mat­ter experts to col­lab­o­rate, share knowl­edge and lead sys­tem-lev­el change. Although host­ed by the CEIH, SCNs belong to the wider health sys­tem and serve as a resource for exper­tise, advice and lead­er­ship in their spe­cial­ty areas. As part of the updat­ed mod­el, we have also cre­at­ed three Pri­or­i­ty Focus Areas to deliv­er ini­tia­tives aligned with sys­tem pri­or­i­ties. We are work­ing with part­ners and project teams on clin­i­cal reg­istries and sep­sis improve­ment, with updates avail­able on our web­site as these projects evolve. 

Rheumat­ic Fever Pri­or­i­ty Focus Area project 

The CEIH has also begun ear­ly scop­ing for our project on rheumat­ic fever. This pre­ventable con­di­tion con­tin­ues to have sig­nif­i­cant impact on First Nations peo­ple. Reduc­ing the inci­dence of rheumat­ic fever is an impor­tant part of improv­ing equi­ty and clos­ing the gap in health out­comes for Abo­rig­i­nal people.
