Primary Care Statewide Clinical Network

Coming soon!

About the network

The Primary Care Statewide Clinical Network (PC SCN) is a new network in South Australia focused on strengthening primary health care. It builds on the work of the pre-exiting primary health care subcommittee and will bring together clinicians, primary care practitioners, consumers and stakeholders from across the state.

The network aims to strengthen primary health care across South Australia by improving access, integration with hospitals, and preventative care strategies.

Why this matters

Primary care is usually the first point of contact for most patients and a key part of a coordinated health system. Strong primary care helps improve equity, reduce avoidable hospital admissions, support people with chronic conditions, and ease pressure on emergency departments.
