Dementia Care Statewide Clinical Network

About the network

The Demen­tia Care Statewide Clin­i­cal Net­work (SCN) is being estab­lished to advance the safe­ty and qual­i­ty of care for South Aus­tralians liv­ing with dementia.

The net­work will bring togeth­er clin­i­cians, con­sumers and sub­ject mat­ter experts to col­lab­o­rate, share best prac­tice and devel­op sys­tem-wide solu­tions to improve demen­tia care.

It will sup­port a per­son- and car­er-cen­tred approach and help reduce unwar­rant­ed clin­i­cal vari­a­tion in care and avoid­able hos­pi­tal presentations.

Why this matters

Demen­tia heav­i­ly impacts old­er Aus­tralians, with care needs becom­ing more com­plex and hard­er to navigate.

Peo­ple need clear­er, bet­ter coor­di­nat­ed care path­ways so they receive the right sup­port at the right time, wher­ev­er they are in the system.
